Does CHAMPVA Require a Referral to See a Specialist?

No. CHAMPVA does not require a referral to see a specialist. You can book a dermatologist, cardiologist, orthopedic surgeon, or therapist directly, as long as the care is medically necessary and the office agrees to bill CHAMPVA. A short list of services needs preauthorization instead, and this guide covers exactly which ones and how to get it.

The short answer, in the VA's own words

You do not need a referral to see a specialist with CHAMPVA.

The VA's provider fact sheet states that approvals for referrals to specialists or for diagnostic tests are not required if they are medically necessary. The sample CHAMPVA ID card in that same document is printed with the words "Open Access, No Referral Required." The VA's consumer page adds that in most cases you do not need to get authorization before you get care.

This is one of the real advantages of CHAMPVA over managed-care plans. There is no primary care gatekeeper and no network, so the question is never "am I allowed to see this doctor," only "will this office bill CHAMPVA." Our provider guide has the phone script for that second question.

Referral versus preauthorization: two different things

A referral is a primary care doctor sending you to a specialist. CHAMPVA never requires one.

Preauthorization is CHAMPVA approving a specific service in advance. CHAMPVA requires it for only a handful of services, listed below. If you get one of those services without approval, the claim can be denied, so this list is worth knowing.

The services that do need preauthorization

The VA's current consumer list has four items:

  • Inpatient mental health care, meaning a stay at a facility.
  • Care for alcohol or substance use disorders.
  • Dental care, in the limited cases where CHAMPVA covers dental at all.
  • Organ transplants.

The VA's provider fact sheet adds two more that offices should know about: bone marrow transplants, and durable medical equipment with a purchase price or total rental price of $2,000 or more. Certain prescriptions also carry a prior authorization: Wegovy and Zepbound, which CHAMPVA covers only for approved medical conditions and never for weight loss.

Everything else is open access. Routine outpatient therapy, specialist visits, imaging, lab work, surgery, and maternity care do not need advance approval. One exception worth knowing: if you get medical equipment or mental health care at a VA facility under the CHAMPVA In-house Treatment Initiative, no approval is needed even for the items above.

  • Email: the provider fact sheet lists VHAHAC.PreauthorizationFM@va.gov.
  • Who asks: usually the treating provider's office submits the request, because it needs clinical details. Ask the office to handle it and to give you the approval reference before the service date.

What if the specialist's office insists on a referral?

Some practices require a referral as their own office policy, no matter what insurance you carry. CHAMPVA cannot stop a practice from having that rule. If you hit it, you have two easy options: ask your primary care provider to send a quick referral note, or pick another specialist from our directory. Either way, CHAMPVA itself will not deny the claim for lack of a referral.

Front desks also sometimes confuse "referral" with "authorization." If a scheduler says CHAMPVA needs authorization for a routine specialist visit, ask them to check with billing. The answer, for anything not on the list above, is no.
